Health insurance is more complicated than in-network status. Here are some other elements that are necessary for treatment to be covered by your health plan.

• Medical Necessity - the client must meet criteria for a diagnosis to qualify for treatment coverage

• Authorization - some plans require prior approval before services can begin.

• Claim Submission - after services are provided, the therapist must provide information to the health plan including the diagnosis code, type of treatment provided.

• Clinical Documentation - sometimes a health plan may ask for clinical documentation including treatment plans and session notes.
